Driver Qualification File (DQF) Setup & Audit

A Driver Qualification File (DQF) is the FMCSA-required record set that documents a driver's eligibility to operate a commercial motor vehicle, maintained for the duration of employment.

What It Is

DQFs are required under 49 CFR Part 391 and must be maintained for every CDL driver operating under the carrier's authority.

Files must include the driver's application, MVR, road test or equivalent, medical certificate, and annual review records.

Incomplete or missing DQFs are one of the most commonly cited violations in FMCSA compliance reviews.

  • Carriers converting from independent contractor to employee-driver models

Requirements Checklist

  • Motor Vehicle Record (MVR) — initial and annual
  • Road test certificate or equivalent
  • Medical Examiner's Certificate
  • Annual review of driving record

Timeline

Initial file setup within 3–5 business days per driver; ongoing annual review scheduling included.
